Claude Loses Track of Conversation: How to Fix

As conversation threads with Claude grow longer, you may notice the model starting to forget earlier instructions, ignore specific constraints, or hallucinate details. This behavior occurs because of context window limitations and attention dilution. Even though Claude models have large context windows (up to 200,000 tokens), the model's ability to recall information from the middle of a very long prompt decreases—a phenomenon known as "lost in the middle."
Here is how to resolve context drift and keep Claude on track during long-form work.
1. Start a Fresh Conversation Thread The most effective way to restore Claude’s performance is to start a new chat. When a thread contains thousands of words of history, Claude has to process the entire log with every new message. This not only slows down response times but also dilutes the weight of your original instructions.
- Export key information: Before abandoning the thread, ask Claude: *"Summarize our progress, the agreed-upon rules, and the current state of our project in a concise list."*
- Paste into a new chat: Copy that summary and use it as the starting prompt for your new conversation.
2. Structure Your Prompts with XML Tags Claude is trained specifically to recognize and prioritize information enclosed in XML tags. If you dump raw text or instructions into a long thread without structure, Claude treats it all as flat text and may lose track of what is an instruction versus what is reference data.
Use clear, nested XML tags to separate your directives:

This structure helps Claude's attention mechanism isolate the active rules from the data it needs to process.
3. Pin Core Files Using Projects (Claude Pro) If you are using Claude Pro or Team, do not repeatedly paste the same reference documents, style guides, or codebase files into your chat messages. This quickly fills up the active context window and degrades performance.
- Navigate to the Projects dashboard on Claude.ai.
- Create a new project and upload your reference files to the Project Knowledge sidebar.
- Start your chats inside this project.
This ensures that Claude reads the reference material as a system-level grounding source rather than reprocessing it as part of the conversational back-and-forth.
4. Reinforce Instructions mid-Conversation If starting a new thread is not an option, you must actively guide Claude's attention back to your core requirements. Every 4 to 5 messages, insert a brief reminder of the rules.
- Example: *"As a reminder, do not use external libraries for this task. Continue with the next step using only standard Python libraries."*
- This pulls the constraint back into the most recent part of the context window, where Claude's attention is strongest.
5. Limit the Scope of Your Prompts Asking Claude to perform ten complex steps in a single prompt increases the likelihood of error. Break your workflow down into smaller, sequential tasks.
- Ask Claude to complete Step 1.
- Review the output and approve it.
- Prompt Claude: *"Now, using the output from Step 1, proceed to Step 2."*
This keeps the immediate attention window focused entirely on one micro-task at a time.
When to escalate If Claude continues to lose track of basic instructions within the first two or three messages of a completely fresh thread, there may be an active platform-wide quality regression or an issue with your browser's local state. Check the official Anthropic Status page (status.anthropic.com) to see if there are any active API or model performance degradations. If the status is normal, try clearing your browser cache or switching to a different web browser to rule out script execution lag.
