How to Fix Claude Giving Wrong Answers Suddenly

1. Start a fresh chat session The most common cause of sudden accuracy loss is context window saturation. As a chat session grows longer, Claude has to process thousands of tokens of previous conversation. This can lead to "attention drift," where the model prioritizes older, less relevant parts of the chat over your latest instructions.

  • The Fix: Export your current chat if you need to keep a record, then click Start New Chat. State your core instructions fresh in the first message. Keeping chats short and single-purposed drastically improves reasoning accuracy.

2. Isolate instructions using XML tags Claude is trained specifically to understand structure. If your prompt mixes instructions, reference data, and questions together in a single block of text, the model can get confused and hallucinate wrong answers.

When to escalate If Claude continues to provide completely broken, incoherent, or wild answers across multiple new chats, there may be an active backend outage or a flawed model deployment.
