ChatGPT Giving Bad Answers? How to Fix Quality Drops

Updated 10/2/2026

If you have noticed that ChatGPT is suddenly giving worse answers, generating incorrect code, acting "lazy," or ignoring your explicit instructions, you are experiencing what is often called model drift or quality regression.

While OpenAI frequently updates its models to improve speed and safety, these changes can sometimes alter how the AI interprets complex prompts. Additionally, account-level settings like accumulated memory or long chat histories can corrupt the model's output over time.

Follow this step-by-step guide to diagnose and fix poor output quality in ChatGPT.

1. Clear or temporarily disable Custom Instructions Custom Instructions tell ChatGPT how to behave across all chats. However, instructions written for older models (like GPT-3.5 or GPT-4) can conflict with newer updates (like GPT-4o), leading to repetitive, overly brief, or highly structured but low-quality answers.

  1. Click on your profile picture or name in the bottom-left corner of the ChatGPT interface.
  2. Select Customize ChatGPT (or Custom Instructions).
  3. Copy your current instructions and save them in a local text file for backup.
  4. Delete all text in both the "What would you like ChatGPT to know about you?" and "How would you like ChatGPT to respond?" fields.
  5. Toggle off Enable for new chats if the option is present, then click Save.
  6. Start a brand-new chat and test your prompt again.

2. Manage and clear the Memory feature ChatGPT's "Memory" feature allows it to remember details across all conversations. Over time, it can store conflicting or outdated preferences that degrade the accuracy of your current tasks.

  1. Go to Settings > Personalization > Manage Memory.
  2. Review the list of stored memories.
  3. Click the trash can icon next to any memories that are vague, outdated, or irrelevant to your current workflow.
  4. Alternatively, toggle the Memory switch to Off completely to see if output quality improves without background memory retrieval.

3. Avoid "chat fatigue" by starting fresh threads ChatGPT relies on the context of your current conversation to generate answers. As a chat thread grows longer, the context window fills up. To keep processing your prompts, ChatGPT must summarize or "forget" older parts of the conversation, which frequently leads to hallucinations, repetitive sentences, and lazy coding blocks.

  • Rule of thumb: If a chat thread exceeds 15 to 20 exchanges, or if you notice the model failing to remember details from five prompts ago, click New Chat.
  • If you need to carry over complex instructions, copy-paste a concise summary of the project state into the first prompt of the new thread.

4. Force "Chain-of-Thought" prompting Newer OpenAI models are optimized for speed, which can sometimes cause them to rush to an answer without processing intermediate logic. You can force the model to slow down and verify its work by using structured prompting techniques.

  • Add reasoning steps: Append this phrase to your prompt: *"Think step-by-step before answering. Write out your reasoning process in a 'Draft' section before presenting the final solution."*
  • Apply negative constraints: Tell the model what to avoid. For example: *"Do not use placeholder code or write '// insert code here'. Write out the entire functional code block."*

5. Switch model versions If you are a ChatGPT Plus or Team subscriber, you have access to multiple models. A system update might temporarily degrade performance on one specific model.

  1. Locate the model selector dropdown at the top of your chat window.
  2. Switch from GPT-4o to GPT-4 (or vice versa). GPT-4 is slower but often handles highly complex logic and long-form coding with fewer errors than the faster GPT-4o.
  3. For basic tasks, test GPT-4o-mini to see if a lighter model resolves the issue.

When to escalate If ChatGPT continues to generate broken formatting, raw JSON errors, or completely nonsensical text despite starting a clean chat with no memory enabled, the issue is likely a platform-wide outage or a bad deployment.

  • Check the official OpenAI Status Page (status.openai.com) to see if "API" or "ChatGPT" services are experiencing active incidents.
  • If the service is operational but your account is stuck in a degraded state, click the Help (question mark) icon in the bottom-right corner of the web interface to submit a bug report directly to the engineering team.
