Claude Keeps Apologizing and Repeating Errors: How to Fix

Updated 9/5/2026

Why Claude Gets Stuck in Apology Loops

It is a common and frustrating behavior: Claude makes an error in its code or writing, you point it out, and the model responds with: *"You are entirely correct. I apologize for that oversight. Here is the corrected version."* However, the new output contains the exact same bug or stylistic mistake. If you point it out again, the loop repeats.

This behavior stems from two factors: 1. Sycophancy Bias: Large language models are trained to be polite and agreeable. They will readily admit to mistakes, even when they do not fully understand how to correct them. 2. Context Window Reinforcement: Every time Claude generates a response (even a wrong one) and you reply to it, that entire exchange is fed back into its context window. The model sees its own previous incorrect code as a strong statistical pattern to follow, overriding your corrective instructions.

To break this cycle, use the following structured troubleshooting steps.

---

Steps to Stop the Apology Loop

1. Clear the Context Window (Start a New Chat) The fastest and most effective fix is to abandon the current chat thread. Once Claude has repeated an error twice, its context window is polluted. * Copy your original prompt, the correct parts of the output, and your specific requirements. * Click **"New Chat"** in the top left corner of the interface. * Paste the clean instructions into the new window. This wipes the history of the error, preventing the model from being anchored to its previous bad output.

2. Instruct Claude Not to Apologize Claude wastes tokens and reasoning power on polite preambles. Force the model to focus on raw analysis by explicitly banning apologies. * Add this instruction to your prompt: *"Do not apologize or explain what went wrong. Simply output the corrected code/text directly. If you understand the correction, write 'Understood' and provide the solution."* * By removing the conversational filler, the model dedicates its entire generation sequence to solving the technical problem.

3. Use XML Tags to Isolate the Error and Solution Claude is highly sensitive to structure. Using XML tags separates the broken code from the fix, which helps the model focus its attention. Structure your corrective prompt like this:

`xml <broken_output> [Insert the incorrect code or text Claude just generated here] </broken_output>

<error_description> [Explain exactly why this is wrong. E.g., "The function on line 12 is using deprecated syntax."] </error_description>

<corrective_action> Rewrite the code inside the tags above. You must replace the deprecated syntax with the updated library methods. Do not reuse any lines from the <broken_output> block that contain the error. </corrective_action> `

4. Apply Few-Shot Prompting If Claude keeps failing to correct a specific logic error, show it what a correct fix looks like. Provide a miniature, simplified version of the problem and the correct resolution. * Example: *"Here is an example of how to resolve this specific issue: [Insert input -> output example]. Now, apply this exact same logic to my request below."*

5. Lower the Complexity of the Prompt Apology loops often happen when Claude has reached its cognitive limit for a single prompt. If you have loaded a massive file and asked for multiple changes at once, Claude may struggle to track them all. * Break your request down into micro-tasks. * Instead of asking it to "fix the bugs and optimize the performance of this script," ask it to: *"Step 1: Fix the syntax error in the database connection string. Do nothing else yet."*

When to Escalate

If Claude continues to generate identical incorrect loops across entirely fresh chat windows, or if it fails on basic, previously successful prompts, there may be an underlying model degradation or an issue with Anthropic's serving infrastructure. Check the official Anthropic Status page (status.anthropic.com) to see if there is an active incident report for "degraded performance." If the status is green and the issue persists across different browsers and accounts, you can file a bug report via the in-app help widget or contact Anthropic Support directly.
